Why the brand matters at all
Not because of success rates. Published survival figures for well-documented systems sit close together, and the implant that fails is almost always the one placed in the wrong position or in insufficient bone rather than the one with the wrong logo.
The brand decides two things: how much independent research stands behind the implant surface, and how easily a dentist in your own country can service the restoration in ten years. The second is the one patients underestimate.
Straumann
Swiss-made, with the SLA and SLActive surfaces. Few implant systems have a comparable body of independent published follow-up behind them. Prosthetic parts are stocked almost everywhere in Europe, which makes long-term servicing straightforward wherever you live. Positioned as premium, and priced accordingly.
Nobel Biocare
Originally Swedish, now headquartered in Switzerland. Nobel Biocare helped establish modern implantology; the TiUnite surface is documented in large long-term studies. Parts availability across Europe is equally good. Also premium.
Megagen
South Korean, with its own surface technology and a broad prosthetic range. The published literature is substantial and growing, and the system is positioned at high volume rather than premium pricing. Parts are obtainable across Europe, though availability varies more by country than it does for the two Swiss-based systems.
What actually separates them
Surface technology. All three use proprietary, documented surfaces. This is where the research money goes and where the differences are real but modest in clinical outcome.
Independent long-term data. Straumann and Nobel Biocare have the deepest archives. Megagen has a large and growing one. All three pass the test of being checkable, which the value tier generally does not.
Prosthetic versatility. The wider the abutment range, the better a difficult implant position can be restored. All three are workable; the premium systems have the deepest catalogues.
Parts in your own country. Excellent for Straumann and Nobel Biocare, good for Megagen. For an unbranded or regional system, often impossible — which is the real reason to avoid the bottom of the market.

What to put in writing, whichever brand you choose
The brand only protects you if it is documented. Four details, for every implant placed, before you leave the clinic: brand, diameter, length and lot number.
Every implant ships with a lot-number sticker. Ask for it to be attached to your treatment record, not read out to you. With those four details any dentist in your country can order the correct abutment or screw a decade from now. A clinic that will not give you this is a warning sign, and it is a cheap thing to ask for.
Add two more to the same document: the guarantee period for the implant and separately for the restoration on top of it, and a named contact for problems after you are home.
What decides the outcome instead
If the brand is the fourth most important factor, these are the first three.
Position. An implant placed where the bone is, angled so the tooth above it can be made properly, in a position that can be cleaned. Get this wrong and no brand recovers it.
Bone volume. Enough bone around the whole circumference, assessed on a CT scan before surgery rather than discovered during it.
Soft tissue. The gum cuff around the implant is what keeps bacteria out of the bone. It is the part of implant work that shows most in the mouth and gets discussed least in quotes.
When you are comparing clinics, these are the things to ask about. A clinic that talks only about brands is selling you the easiest part of the decision.
Which is right for you
You want the deepest research archive and the widest parts network: Straumann or Nobel Biocare. You are paying for documentation and logistics, and both are real things to pay for.
You want a well-documented system without the premium: Megagen. Check with a dentist at home that parts are obtainable in your country, then proceed.
You are being offered something you cannot find independent data on: decline, or ask what the plan is when a part is needed in ten years.
In every case: the surgeon decides more than the brand. Position, bone volume and soft-tissue management are what determine whether the implant lasts.
